RESPONSIBILITIES

“TEMPORARY” ASSIGNMENTS:

LAB FORCE places Professionals in “Temporary” assignments, when the work is complete, the assignment is finished.  End dates for assignments may vary as the work load changes.  Your flexibility in meeting the needs of our Customers may result in increased work opportunities for all of us! 

 

 

ATTENDANCE

When you accept an assignment, make sure prior commitments do not interfere with your ability to complete the assignment.  Your on-the-job attendance underscores YOUR importance as a Professional, as well as the importance of our Customer's Patients who are in need of your fine services. 

·         Be on time for your assignment.  It shows respect for your work and for our customers.

·         48 hours’ notice is required if you cannot complete a given assignment.

·         24 hours’ notice is required if you cannot complete a particular day’s shift.

·         Notify your supervisor or department if your attendance will be interrupted or delayed.

·         Notify your Staff Coordinator at LAB FORCE if your attendance will be interrupted or delayed.

 

QUALITY ASSURANCE

Your Staff Coordinator will conduct a “Quality Assurance” check with YOU within 24 hours of beginning an assignment.  Any concerns or questions you have should be discussed at that time.  Your Staff Coordinator will also conduct a “Quality Assurance” check with our Customer to be sure all needs are being met.  Additional “Quality Assurance” calls will be made to the Customer at the two week and four week points.  Prompt attendance is key to the successful continuation of assignments.

 

If you miss a day’s assignment without giving proper or emergency notice so that a replacement can be found in a timely manner, your continuation in the assignment will be jeopardized.   The Customer’s decision about your continuation is final.

 

 

EMPLOYMENT

Remember that you are a LAB FORCE employee.  Your introduction to our customers through the completion of assignments are managed by your LAB FORCE Staff Coordinator.  LAB FORCE will issue your weekly paychecks based on the Time Cards you submit, and will deduct appropriate local, state and federal taxes.

 

 LAB FORCE provides Worker’s Compensation coverage, General and Professional Liability insurance coverage for all “Temporary Assignment” employees.

LAB FORCE SERVICES reserves the right to amend, modify or change the enclosed policies and benefits at any time.

 

ATTIRE

First impressions are important!  Dress to conform - not to stand out.  Remember to follow Professional requirements  when dressing for an assignment and ALWAYS follow the customer’s dress codes. Your Staff Coordinator will have this information for you. Professional attire dictates conservative clothing, and moderate hair and/or make up styles.   Be sure to follow OSHA requirements for gloves, safety glasses, etc.

 

 PROFESSIONAL BEHAVIOR

Avoid personal phone calls when on the job.  In the case of an emergency, if a personal call must be made, call from a pay phone during a break or, if necessary, ask permission from your supervisor to have time to make a call.  Be sure to keep PERSONAL problems away from work!

 

Discuss work problems, salary questions and/or offers of permanent employment WITH YOUR STAFF COORDINATOR at LAB FORCE - not with your assignment supervisor.  Remember that LAB FORCE is YOUR EMPLOYER.  Do not discuss employment issues with the customer.

 

 

UNLAWFUL HARASSMENT

It is LAB FORCE’s intention to maintain a work environment free from all forms of discrimination and harassment, including sexual harassment.  If you believe you have been subjected to harassment you should contact LAB FORCE SERVICES central offices immediately.  All complaints will be investigated and treated with the utmost confidentiality.  Violations of this policy by LAB FORCE SERVICES employees will result in discipline up to and including discharge.  For additional information, please contact your Staff Coordinator.

 

 

DRUG AND ALCOHOL ABUSE

It is the policy of LAB FORCE SERVICES that the use, possession, transfer or sale of illegal drugs or controlled substances by Temporary Assignment Employees is prohibited.  Consequences may include disciplinary actions, up to and including termination. Alcohol and drug testing may include pre-employment, upon reasonable suspicion,  for cause, post accident, randomly,  periodically or at the request of the customer.

 

HIPAA and JACHO Standards

 

All Lab Force Services Temporary Assignment Employees will render care in accordance with the ethical and professional standards, policies and procedures, all applicable statutes and regulations, including but not limited to the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the requirements of the Joint Commission on Accreditation of Health Care Organizations (“JCAHO”) and other regulatory agencies to which our customers are subject.

 

All Lab Force Services Temporary Assignment Employees will respect the confidential nature of all PHI (Personal Health Information) relating to patients.  Further, Lab Force Employees shall comply with all applicable laws, rules, and regulations governing confidentiality, use and disclosure requirements of the federal, State and local laws and regulations, including but not limited to those provisions concerning HIV/AIDS-related information of AIDS and Immunodeficiency Virus and HIPAA.  

 

All Lab Force Services Temporary Assignment Employees warrant that they have not been excluded from participation in federal health care programs.

 

CONFIDENTIALITY

As a LAB FORCE employee on “Temporary” assignment to our Customers, you will be exposed to both technical and non-technical information; business methods and practices; pricing,  medical records and other such information not generally available to the public.  All LAB FORCE Temporary

Assignment employees agree to keep secure and confidential any proprietary (technical and non-technical) information either during or subsequent to employment.

 

Your Staff Coordinator may request that you read and sign specific Confidentiality Agreements with certain of our Customers.  Such Agreements are required to be assigned to that Customer.

 

SET A POSITIVE AND PROFESSIONAL EXAMPLE:

Before making a personal comment or asking personal questions of another worker or Patient, think:  “Could my comments or questions make another person feel uncomfortable?” 

 

Avoid making assumptions that “practical jokes”, “friendly gestures” - both verbal and physical - are harmless or inoffensive.  Quite often such jokes and gestures are not viewed as inoffensive by others.

Set a positive example!  Treat everyone with respect and let others know that you expect the same treatment.

 

Finally, be considerate of both the physical and emotional aspects of your Patients’ privacy and confidentiality.

 

 

RIGHT TO KNOW

Employers are required to provide their employees with information and training regarding hazardous chemicals and materials in the workplace.  The company you are assigned to is responsible for identifying such items, labeling containers, providing training on the safe use of hazardous chemicals and securing Material Safety Data Sheets. You have the right to review these sheets as they provide valuable information for helping you avoid injury or illness while handling hazardous substances.

SAFETY

Your safety on the job is of the utmost importance to LAB FORCE.  Please take the time to learn the proper safety procedures at your assigned location and follow general rules of safety at all times.  Should an accident occur, notify your supervisor immediately, and call the Safety Officer at the LAB FORCE Central Offices at 1-800-522-8324.

 

 

PAYROLL PROCEDURES

·         At the end of your work week, complete and sign a TIME CARD.  Have your supervisor sign the card to verify the hours worked. 

·         Be sure that you clearly write in the DATES of the days you work.  If requested, include your Job Number and Social Security number.

·         Fax your Time Card to LAB FORCE Financial Service Center at: 603-893-5191.

·         Paychecks are mailed twice each week: Time Cards received by 10am Monday will be processed and mailed Tuesday.  Time Cards received by 10am Wednesday will be processed and mailed Thursday.

·         LAB FORCE reserves the right to make reasonable adjustments to the payroll schedule, as noted above, during holiday periods.

·         Your paycheck will be mailed to the address which appears on your W-4.

·         LAB FORCE offers Direct Deposit service. The enclosed enrollment form explains the benefits and procedures.

·         Most Direct Deposits are credited within two working days of payroll being processed.  Check with your bank to see how quickly they credit Direct Deposits.

·         A new Time Card is mailed with each paycheck.

 

WHEN TO CALL

LAB FORCE

As a Team member, you  are asked to communicate with your LAB FORCE Staff Coordinator on a regular basis.  Your input on the experience gained during your assignments is valuable in helping us to do a good job for YOU!  The LAB FORCE Central Office hours are Monday through Friday, 8am to 6pm EST.  Our toll free number is 1-800-522-8324

 

Professional Changes:

·         If you change your professional status,  certification and/or licensure, LET US KNOW!

·         If you need additional hours or a change of schedule, LET US KNOW!

·         If you are planning to move and would like to pursue career opportunities in a new location, LET US KNOW!

 

Payroll Questions:

·         If you do not receive a paycheck in a timely manner, please give us a call.

·         If you change your name, address, telephone number, and/or tax exemptions, let us know. We require that you fill out a new W-4 form.

 

Emergencies:

·         24 hours’ notice is required if you are going to miss a day of work. 

·         In case of an emergency that will cause you to miss work or arrive later than expected, you are required to contact your department supervisor at the job site AND LAB FORCE.

·         If you have an “on the job” accident, immediately call LAB FORCE and speak to the Safety Officer.  Because LAB FORCE is concerned about your health and safety, and also manages your Worker’s Compensation insurance, timely reporting of an accident is essential to your benefit coverage.

 

 

GENERAL QUESTIONS

·         If you experience problems or have concerns while on assignment, please contact your Staff Coordinator DURING A SCHEDULED BREAK IN YOUR WORK RESPONSIBILITIES!

·         If you are asked to use your vehicle or the customer’s vehicle for travel during an assignment, immediately call so that appropriate paperwork can be transmitted and signed.  This paperwork assures your insured protection and the customer’s.

·         When your assignment ends or is extended by the customer, be sure to call your Staff Coordinator.
